Here are some tips and guidelines to help you fill out a health questionnaire in Spain:

1. Personal Information

• Full Name (Nombre completo): Write your complete name, including first name(s) and surname(s). In Spain we have 2 surnames so you might find two spaces, only use the second one if you do have a second surname.
• Date of Birth (Fecha de nacimiento): Include the day, month, and year, in that order.
• Id number (DNI/ NIE/ Pasaporte): Provide your official identification number, whether it’s your Spanish DNI (for residents), NIE or passport number (for non-residents).
• Contact Information (Información de contacto):
• Phone number (Número de teléfono): Include your home phone number.
• Email address (Correo electrónico): Provide your email address, if applicable.

• You will be required to inform our support team as soon as you have your NIE, Spanish address and Spanish telephone number so that we can update your files.

2. Medical History

• Pre-existing Conditions (Enfermedades preexistentes): List any chronic conditions or past serious illnesses you have ever been diagnosed with, such as diabetes, hypertension, asthma, etc.
• Allergies (Alergias): Note any known allergies (e.g., food, medications, environmental allergens).
• Surgeries or Major Treatments (Cirugías o tratamientos importantes): Provide details on any previous surgeries, hospitalizations, or treatments you have undergone.
• Medications (Medicamentos): List any prescription or over-the-counter medications you are currently taking, including dosages and frequency.

3. Current Health Status

• Current Symptoms (Síntomas actuales): Describe any health issues you are currently experiencing, such as fever, cough, shortness of breath, pain, etc. A medical report might be asked for, English will be accepted.
• Weight and Height (Peso y altura): Fill in your current weight and height in kilograms and centimeters, respectively.
• Blood Pressure (Presión arterial): If required, provide your latest blood pressure reading (if known).
• Eyeseight (dioptrías): If required, provide your latest diopter reading, one for each eye, or 00/00.

5. Lifestyle Information

• Diet and Exercise (Dieta y ejercicio): You may be asked to detail your eating habits, regular physical activity, or any other habits that impact your health (e.g., smoking or alcohol consumption).
• Smoking (Fumar): Indicate whether you smoke, how many cigarettes per day, or if you have quit.
• Alcohol Consumption (Consumo de alcohol): Provide details about your alcohol consumption frequency and quantity.

6. Signature and Date (Firma y fecha)

• Signature (Firma): Sign the form to confirm the accuracy of the information provided.
• Date (Fecha): Write the date you filled out the form following this format: In “city”, el “day” de “month” de “year”.

7. Bank details (datos bancarios)

You will not be required to provide any bank details unless informed otherwise through the application or support team. Payment will always be made by card or transfer on the first year then you will have to update your IBAN number as soon as you’ve opened a bank account in Spain.

8. Attaching documents (adjuntar documentos)

Accepted file types: jpg, jpeg, png, pdf.

9. Submission

Online Submission: Depending on the company, some extra info might be required, like a medical report or maybe some information did not fit in the provided document, you can send us an email with the extra information and medical reports.
